Ordering a P.E.T./CT Scan Made Simple

This page provides guidance for ordering a P.E.T./CT Scan with us.
Inside, you’ll find a P.E.T./CT Order Form and Patient Preparation Instructions to help ensure a smooth and efficient referral process.

We make the ordering process easy for your office:

1.

Complete the P.E.T./CT Order Form.

2.

Attach the following documents:

  • Patient demographics
  • Recent clinic notes
  • Recent imaging reports
  • Pathology reports 

3.

Fax all documents

to the preferred location listed at the top of the order form.

Once we receive the referral, our team will:

  • Contact the patient directly to schedule their P.E.T./CT Scan.
  • Obtain any required prior authorizations from the patient’s insurance provider.

After the P.E.T./CT Scan is completed, a radiologist will interpret within 24 hours, and the final report will be faxed directly to your office.
