Ordering X-Ray Services Made Simple

This page provides guidance for ordering X-Ray Services.

Inside, you’ll find a X-Ray Order Form and options for submitting the form to our office.

No appointment is necessary — simply complete the order form and:

Send your patient with the order form

for a same-day X-ray.

OR

Fax the order form

if the patient will come later, and we will contact them to schedule.

After the X-Ray is completed, a radiologist will interpret within 24 hours, and the final report will be faxed directly to your office. Please be sure to include your office fax number on the order form.

We perform X-rays Monday through Friday, 8:00 a.m. – 3:30 p.m.
